SUMMARY OF JOB RESPONSIBILITIES
Responsible for the overall leadership, management and direction of non-hospice, post-acute clinical operations according to state and national regulations and consistent with the organization’s mission, vision and core values. The Vice President, Post-Acute Care provides the leadership, management and vision necessary to ensure the organization and service lines have the proper operational controls, administrative and reporting procedures and people systems in place to effectively ensure operating efficiency and viability. The Vice President will assist the CEO and COO, Clinical Services of Bluegrass Care Navigators in providing overall leadership, strategic planning, evaluation, potentials for growth and improvement and development of healthcare provider and payer and community relations.
RESPONSIBILITIES AND DUTIES
Note: During the first year of employment, the incumbent will be responsible and focus on the start-up of PACE operations. Supervision of non-hospice, post-acute clinical services will transition from the COO, Clinical Services to the Vice President, Post-Acute Care as PACE personnel are hired and start-up of PACE operations is complete.
Ensure that all program activities operate consistently and ethically within state and national regulations, accreditation standards as well as the mission, vision, and values of the organization. Assures compliance with all licensing, certification, accreditation and regulatory standards.
Actively develop and mentor each leader of non-hospice, post-acute clinical services. Provide day to day support and oversight of the post-acute leadership team.
In conjunction with Bluegrass Care Navigators leadership, promote, develop, and nurture strategic relationships with hospital, nursing home, physician, payer, and professional referral source executives.
Oversee operations of post-acute clinical services at all regional offices, including in-patient facilities, nursing homes or other locations. Overall coordination of direct and indirect post-acute services provided by clinical personnel of the organization.
Establish and monitor performance reporting systems, managerial systems, policies, procedures, and controls necessary to ensure effective and efficient functioning.
Ensure successful, high-quality, and efficient transition of patients along the agency’s continuum of care.
To support decision making, use data and analytics to develop improvement strategies and identify growth opportunities.
Ensure effective allocation, utilization and return on resources, including staff, space, equipment, technology and capital.
Assure prompt response to concerns and complaints of staff, patients and/or families, partners and payers and participates in problem solving and service recovery as needed. Assure root cause analysis and resolution of service concerns.
Assist in the preparation and administration of the agency's budget. Manage budget development and implementation strategy, timelines and resource allocation. Ensure the continued financial viability of services through sound fiscal management. Use financial data to monitor costs and recommend cost management strategies.
Identify opportunities to decrease waste and increase the quality of service by simplifying work processes.
Oversee compilation, maintenance and interpretation of operational indicators to detect census changes, increases or decreases in volume, which could impact staffing levels, revenues, expenses and resource utilization. Assure Executive Directors, Directors and Managers use established metrics in day-to-day operations.
Ensure staff are fully informed on operational objectives. Empower staff by providing needed information and encouraging and facilitating team participation in decision-making.
Build commitment to change by influencing and educating others, personally embracing the need for change, mentoring and facilitating planned change, monitoring the change process and evaluating the impact of change on staff and patients. Recognize, understand and respond to others reactions to change in ways which are supportive and help to reduce resistance to change while gaining commitment.
Hire and supervise an effective clinical management team. Implement appropriate leadership development that ensures achievement of agency goals. Evaluate performance of leadership and supervisory personnel per the organization’s policy. Assist leadership and supervisory personnel in developing skills and techniques in evaluating clinicians' performance.
Evaluation of organization performance via quality management program, productivity, monthly, quarterly and annual review. Assure the quality and safe delivery of services provided.
Stay informed about changes in health care; shares information with appropriate agency personnel and administrative personnel.
Promote Bluegrass Care Navigators in the health care community; maintain a positive community image and assure leadership for increasing awareness and understanding of Bluegrass Care Navigators programs and services.

Master’s degree in Nursing, Social Work, Public Health, Health Administration, business administration or similar degree from an accredited program preferred. Bachelor’s Degree with relevant and tenured work experience considered.
Five to ten years of supervisory experience in a health and/or service-oriented environment is preferred with progressively responsible experience in supervisory or administrative duties or a combination of education and supervisory experience.
Understands post-acute healthcare and has knowledge of healthcare regulations, financing, and trends.
Experience with issues concerning serious illness, chronic illness and the needs/expectations of payer, partners and providers.
Possesses and maintains good physical stamina and mental health. Has presented a pre-employment physician's health clearance including a negative TB skin test and/or CXR and other tests as required by organization policy.
