Thresholds Health is seeking a dynamic leader to launch, lead, and grow a new, full-service primary care health center for members of West Side communities in Chicago. Thresholds Health will deliver compassionate, integrated primary, mental health, and substance use care to address complex healthcare needs and to achieve better health outcomes.  The CEO of Thresholds Health will oversee all aspects of this start-up enterprise, as well as its application to become a Federally Qualified Health Center Look Alike (FQHC LAL). The CEO is responsible for the overall leadership and management of Thresholds Health and will manifest an organizational vision to increase access to care, fight structural racism, and create equity in communities. Over time, the CEO will grow Thresholds Health
