The Chief Quality and Population Health Officer (CQPHO) reports to the CEO and is directly responsible for the overall strategic direction and coordination of population health, care management, and outcome components of the medical facilities. The CQPHO will ensure strategy implementation, planning, organization and direction of Quality Improvement and Population Health Initiatives for the organization.
RESPONSIBILITIES:
Oversees MAPs corporate Quality Improvement Plan and the pay for performance programs, including all new incentive programs.
Oversees the advancement and continued improvement of the Quality Assessment and Performance Improvement program, including the appropriate monitoring and evaluation of clinical and administrative systems to ensure effective, high quality, efficient operations.
Serves as the principal leader for population health program development and research, providing strategic vision and direction to ensure innovative models of health care delivery to include the management of the food insecurity program currently in place.
Serves as the organization Risk Manager, responsible for Safety/Risk management for clinical operations, develops/ updates risk management policies as needed.
Provides strategic vision and oversight for creating a culture of safety; identifies and deploys tools necessary to support safety, including but not limited to enhanced systems of internal and external reporting, root cause analysis.
Develops tools to monitor outcomes; specifically, population health indicators such as patient activation, quality of life, disease specific clinical outcomes and health risks.
Provides consultation, motivation and technical assistance to leaders and other staff to identify the most effective ways to organize, manage, finance, and deliver high-quality care.
Prepares various reports to present findings and to highlight successes and opportunities for improvement or intervention.
Develops and implements analysis and reporting programs for the organization and other population health management activities by ensuring data aggregation, measurement, analysis, and reporting to support improvement and redesign.
Creates and maintains up to date workflow and reporting documentation, and acts as a liaison between clinical users and IT management to maximize and support appropriate use applications.
Works closely with the Chief Medical Officer and the executive team, evaluate utilization patterns and costs associated with practice patterns, interventions, implementation efforts, policy changes, and related health services topics.
Oversees patient experience and engagement using surveys and other tools to support data collection, and analysis of data; develop strategies to enhance patient engagement and support a patient-centered approach to care delivery.
EDUCATION/EXPERIENCE:
MD preferred with Population Health Experience; or MBA/MSN/MPH/MPA/MHA or their equivalent and seven (7) years of experience in quality improvement and or Population Health.
Requires in-depth knowledge and understanding of the healthcare market, managed care, capitation, and understanding of the relevant regulations.
Experience with quality improvement programs, integration and informatics related to planning and population health management and involving knowledge of NCQA recognition programs PCMH, Meaningful Use, patient engagement surveys and EMR based reporting.
